How to Report Unusual Incidents or Complaints

Please use this form to report any serious or unusual events to Advocate Access NW. You may also use this form to file a complaint. 

1. Click the blue CONTINUE button at the bottom of the form. You may have to turn your screen if using a phone.
2. Click START. After entering the information, you will be prompted to enter your email address. 
3. Click THE LINK in the email you receive, and the form will be submitted to us.
