To access services from a Direct Support Professional (DSP), you'll need a referral. If you already have a service coordinator or personal agent, simply give them permission to share your information with us by completing the release form below. With your permission, your case manager will share information with us to help us get to know your needs and preferences for support. We will review your support plan documents and ask you questions to ensure we fully understand what you need. You may already have a support worker identified. If not, the best part comes next!
