Accessing services from a Direct Support Professional (DSP) begins with a referral. You have some options to initiate the process. Please read below to explore these options. Always feel free to contact us if you have questions or need further assistance.

With your permission, your case manager will share information with us to help us get to know your needs and preferences for support. We will review your support plan documents and ask you questions to ensure we fully understand what you need. You may already have a support worker identified. If not, the best part comes next!

We offer a range of services including attendant care, relief care, community transportation, and behavior professional services. Ask us about resources for individuals who experience disabilities and their families. Our goal is to empower our clients to live fulfilling and independent lives.


Our services are available to individuals of all ages found eligible to receive services through their community developmental disabilities program (CDDP). To find the CDDP for your county, click here. 


The referral steps are listed below. You can ask us to initiate a referral directly on your behalf (Direct Referral) or contact your services coordinator or personal agent and ask that a referral be sent to us (Self-referral). Our team will guide you through the process and answer any questions you may have.


With a signed release of information, most needed referral documents will be provided by your services coordinator or personal agent. During the review process, we may ask you to provide additional relevant medical information. Our staff can provide a detailed list based on your specific situation.


We provide resources, information, and opportunities for families and caregivers to help them better understand and support their loved ones. Advocate Access NW is all about community, and we encourage and facilitate connections among the individuals we serve, their families, and our staff. We also offer relief care services to give caregivers a much-needed break.


Yes, we do! Parents who qualify under the Children's Extraordinary Needs program can become providers for their own children. This allows parents to be directly involved in the care and support of their children while receiving the necessary resources and compensation. For more information on how to qualify and apply, please contact our office.


Our services are provided at no cost to eligible individuals.

Be sure to include the name of your individual Services Coordinator or Personal Agent in the "Release FROM" section on the first page. If you need a different form or assistance with completing it, don't hesitate to contact us. 
  2. We Make the Request: We will send the release along with a referral request to your Services Coordinator or Personal Agent.
  3. Next Steps: After we receive your referral documents, we will reach out to discuss the services you need and gather any additional information. We will also explain the enrollment process and provide details on the next steps!

  1. Request a Referral: Ask your Services Coordinator or Personal Agent to send a referral packet to Advocate Access NW at referrals@advocateaccessnw.com (503-410-5116).
  2. Complete the Release Form: Your coordinator or agent will help you fill out a release of information. You can still choose to complete the appropriate release form below.
  3. Next Steps: After we receive your referral documents, we will reach out to discuss the services you need and gather any additional information. We will also explain the enrollment process and provide details on the next steps!

Release of Information

The first step in the referral process is to authorize your case manager to share information with Advocate Access NW. Please use the appropriate form below based on your case management service location. Click the blue CONTINUE button at the bottom of the form, then press START to begin filling in the information. After entering the information, you will be prompted to enter your email address. Click the link in the email you receive, and the form will be submitted to us.
